What could cause this?

randomly, the menu above will disappear and users will not be able to access their User CP, Notifications or even log out.

not sure why this is happening or how to fix it, any ideas?

I'm assuming it has something to do with the SCM Music Player I added to the Footer template.

Then remove the music player and see if you still have the same issues.

It's impossible for us to tell if it's the music player without a link to your site and a test user login.
