Help with Logo and header.

Hey guys and gals. I am having a little trouble with my newest style. The header is just blank and i am trying to figure out how to place my logo on it and make it clickable to>forums. i have attached my header to example. Also i have the psd file. Can anyone here change it from 174px to 150px for me? If so i can send the psd file thank you.
