Pre-Sales type form Product

I would like to have a system where users can put in a group of information that is stored in the database. I am asking members to come back and share contact info for a certain group of people. It would almost just be a contact us pre sales type form where I gather
School Name
Name
Email
phone number
website
Club name

Which would then be stored in the database, not just emailed to me. I would love to have different forms, and the ability to categorize them with a hierarchical relationship so I could say have different groups of forms.

I keep needing some variation of this and keep thinking it has to be here somewhere already, but I can't seem to find the right plugin. Anyone familiar with a product that I can use to do this, or do I need to have it custom made.

Thank you so much Ozzy, I like the Advanced Application Forms mod, it is very close to what I was looking for, the only things I would need different is to have more parent / child relationships, and I would love to display the data on the front end.

My original project was wanting to have users share their favorite places to buy supplies in a particular area like on: http://workshopwarrior.com/suppliers.php, You can see how I would want a City, then a group of categories, and then items under categories.

When I realized I also needed to run this project, I realized how similar a plugin the two projects are and it seems I have needed it 3-4 times. I will look more into the other post, it has potential, but from the screenshots I can't figure out where the data came from.
