Is there a way to add an icon image next to user karma rank?

Let me see if I can find something from my old forum... see how it has icons next to the poster based on their karma rating...

How is this karma added to vBulletin? Is it a mod?

No that was from phpbb3. I noticed though that vbulletin has a karma function but did not know if you could add images as well...

Since I have not used 'Phpbbs', I have no idea what it is besides what the word 'kharma' means. Sounds to me that the 'Reputation' system here is pretty similar or could be slightly modified. Since VB does not have a 'karma' feature.
