Looking for Survey for VB4

Hi, i need a survey like hack that allows user to click on a thread, from there it'll transfer them to a new page where they fill out some questions and once they are done. A thread will be published in that format for people with a certain rank to see. Please help asap. :D

I'm looking just about ANY kind of survey software. I was hoping that vbQuiz would do the job, but it's more for forcing a rigid correct answer instead of accounting for subjective opinions.
