Please help: registration spams

My site recently attacked with spam bots (and posts), no matter what I've tried, it keeps happening. Please help.
I've updated my forums to the latest 4x version 4.2.0, and ensured the followings:

- Image verification (GD) enabled
- "Email to friends" turned OFF for moderated user group
- Email verification is ON

Have tried the Question & Answer verification method.. still getting spams.

What I use is the VSa Advanced Forum Rules, where you have to 'Accept'
the terms before you can post your first thread.

It stops all the spam posts on my website, but I still get the bots registering.
Better than nothing I suppose :P

Easelm:
I created a "moderated" user group and put all the new users to this group to moderate their posts, so their posts don't show up, but still, user registration spam is so annoying.
