upgrade issue

Trying to perform an upgrade to 4.19 from 4.02 and when I load the upload.php and enter the user number, I get the heading of the upload page with nothing....meaning no start install button, step list, etc....

We are on a shared ownership setup and I do not have posting privileges on .com so I was hoping to get some help here as I also cannot currently contact the other admin.

Have you tried reuploading all the 4.1.9 files and making sure they all get uploaded completely and then running the script again?

yes. multiple times on multiple browsers.

when looking at the output in IE, it is clear that the html was not fully written as the submit button code is open ended....

I would recommend upgrading to 4.0.8 first. Verify stability, compatible plug-ins, back-up your db again, and then upgrade to 4.1.9.
