I think there are bots that can solve the image verification, so switching to "question and answer" verification might help you. You only need a few simple questions.

i'll try that. I'm also wondering how to ban by IP address.

Because many people have dynamic IPs, use ban by IP sparingly. If you use it too much, you could end up banner legit members trying to register...

Yep Question and answer definably helps. Also approving members manually and emailing the sus ones for more details helps if it gets to that?
