Survey Mod

Hello,

Is there currently a Mod that allows staff to create a survey for members to take? For example, I want to create a survey which asks users how they heard about us? Maybe something similar to this (https://vborg.vbsupport.ru/showthread.php?t=261978) but not a staff application. Any ideas?
