Is there a limit to the number of buttons on the advanced reply menu?

I have added a new bbcode that I would like to have a button for, and the button is not showing up on the advanced reply page.

It does show up on the bbcode page of the admin panel, so I know the button gif is in the correct folder. the size is 21x20 px as specified in the documentation. but it doesn't show up in the advanced reply text menu.

I tried specifying a button for a bbcode I had added earlier and that works fine, so I'm not sure why this new bbcode button doesn't show up.

the gif is not in the page source.

I recently installed the favorite smilies product. I tried disabling the product, but it had no effect on the missing bbcode buton.
