When you click Showgroups and someone doesn't have an avatar, it's replaced with this?

That is the Avatar in the one image it's simply blown way out of proportion. If you've been making changes it's best to go back and review them otherwise if your not experienced and go about trying to customize a style yourself then your bound to run into issues where you've customized one area and did not realize something there you changed affected a alternate location on the site.

Older version style into newer version vB?

Basically unless the author has released an updated style it's going to be quite a few fixes, I would use Firebug and track down the culprits in the CSS and StyleVars ;).
