Users Can't Read Some Threads Past First Page ...

I have gotten complaints from users that they can't read some threads past the first page. I'm using v. 4.0.6 Any ideas?

Maintenance > Update Counters > Rebuild Thread Information

Something (usually a modification) has thrown the post count in the thread table off.
