I had a issue with my folder permissions and with the path to the videos how ever I am using this to show videos in a specific location on my website. I set my Folder to 777 I ignored the error about the brackets and it worked any way

If you changed the image with another one of the same name and overrid the old one then you need to clear your cache before you can actually see it.
Just dump all temp files and then reload.
