How to add a Re3direct Link to all the Links in Forum

I want to have all my links prefixed with some other links.
Like for eg. if I have a link to hxxp://google.com I want it to be converted to

hxxp://mydomain.com/redir.php?url=hxxp://google.com

How can I Do this?
