Prefix issue!!!!

when i create a new prefix group - it creates it and adds it but it is completely blank - no text what so ever - just the field!?

Try disabling your modifications/plugins and then create a new group and see if you still have this problem.
Note: To temporarily disable the plugin system, edit config.php and add this line right under <?php

define('DISABLE_HOOKS', true);
