How do I setup a template for a thread?

Basically what I want to do is, when you click NEW THREAD in a certain section, it will have some stuff already written inside the message box to make life easier.

So an example would be, I create a new thread and in the message box would be:

NAME:
ADDRESS:
CITY:
PHONE #:

Do a search for "stencil" in all mods, titles only and you'll find the stencil mod that does what you want.
