Attachment Manager Broken

I upgraded from vBulletin 3.8.3 to 4.0, and now I see we can't attach files.
When I go to attach a file, click on the Manage Attachments or the icon in the Text Editor, I get nothing. No window, no error. Nothing. (IE 8.0 & Firefox 3.5.7)
I can open attachments already posted.

System Info -
Linux - PHP 5.2.9 - MySQL 5.0.88

Additonal info,
I just confirmed that the icons and buttons in the Text Editor window, and page do not function. I can only type using default text. I can click on radio buttons. Nothing else working.

Create a new style with no parent:

Styles & Templates > Style Manager > Add New Style
Parent Style: No Parent Style
Title: Default vBulletin
Allow User Selection: Yes
Save

Then browse the site using that totally default vbulletin style - do you still have the same problem?

If it is still happening - Try disabling your modifications/plugins and see if you still have this problem.
Note: To temporarily disable the plugin system, edit config.php and add this line right under <?php

define('DISABLE_HOOKS', true);
