Thread Fields

I tried to look for this but had no luck, maybe it isn't even possible.

I wanted to create a forum where if the user creates a thread there would be certain fields to fill out. Is there a way to do this? (objective is to use the information in a database)

(sorry if newbie question...still learning here).

As far as I know, there has been no modification posted yet for vB4 to create a form to submit threads.
