Title doesn't show

Need help, for some reason if I add new infraction to manager the title will not display. Once I create new infraction and place it in title box, click to save.. Shows saved but it's not there.:confused:

So you give infraction to who? Another Administrator?

Try disabling your modifications/plugins, then add a new infraction, and see if you still have this problem.
Note: To temporarily disable the plugin system, edit config.php and add this line right under <?php

define('DISABLE_HOOKS', true);
