Special Form for a specific forum

I want to specify the way users can post in one of my forums
I want them to follow a special form that I specifiy

Like for example
member is looking for
Rent Location:
Duration of rent:
Type of apartment:
...etc

How can Do that ?

Any hack or mod for that?

There is a Stencil Hack which may do what you want (do a search for it).
