Making a Moderator Application!!!

You read the title right. :D I'm making a moderator application to be released here on vbulletin.org :up:

Now, I just need some feedback as to what questions it should include in the application and any features you might want to see in it. It's going to be a pretty easy install for the most part. Just upload the files, import the product and set your settings in vBulletin Options in the AdminCP.

So, lets hear it. What do you want to see in it?

On a more serious note, letting users choose their own questions couldnt be a bad thing :)

I'm thinking about making a radio button option in there as well, more for which position they're applying for. For example:

[] Moderator
[] Super Moderator
