Ongoing issue.. PLEASE HELP

In the almost 18 months I've had a forum we've had this issue and I have no idea why it happens. It's intermittent and seems to have no rhyme or reason.. the problem is people seeing attachments. We've had people fix their browser settings to load images/etc, but it still doesn't work and pops up.

Does anyone have any idea why this happens??

Sometimes there's a link for them to click to view the photo but sometimes there's just that text. It seems to happen to random people and then it "fixes" itself?

It's driving me crazy that I can't fix it. Does anyone know what this is???

do you have vboptimize?
if not, you should get it so you can flush your cache
not saying this would help but it might...
have you went into update counters?
try the attachments....
try updating common counters as well

What is VBoptimize? I don't have it. I will go update counters/attachments/common counters and see if that helps. :)

It's very hard to tell what is going on just with images - you should be looking at the page source. You should check the image paths because they certainly look wrong in the one photo.
