You could use vBCommerce with vBCredits. They have premium addons for real items and for the credits, use cash conversion. It will be like what Sitepoint.com does.

I'm counting members who, like you, are interesting for a real cart for tangible products. It will be free (all my mods are free), and if I see that there are at least 25 members interesting on such a module, I'll have it ready in less than a month, as I've as backend my other mod for downloadable products. So the only that I need to add are: Options per product, Shipping details, Tax details etc

I've post a poll in my site for this topic.
