Form Type Threads

Is there anyway to have a form type post when someone posts a new thread. What I mean is, the user would answer questions to post in this area.

If I had questions like this:


Name:

Address:

Town/City:

State:

Zip Code:



Either the headings would show up when a user starts a new thread in the forum, or when the started a new thread a forum, or a form opens where the user would fill in the blanks.

Have you tried a search on "form*" in "all mods" "titles only"? I have seen a modification that shows a form instead of the new thread box. There is also a Stencil modification that you may be interested in.
