Trouble with avatars

I currently have avatars set to 100x100 and around 120kb in size.

Despite this, some users have avatars that are far beyond these sizes. One user has one that's 320x300 and over 700kb!
How can this happen?

Other users are telling me they cant upload large avatars, so it cant just be a simple permissions error.

Make sure that all usergroups are set to this limit. If a user is in a second usergroup that has a larger limit, they will be able to have a larger avatar. Also, if you reduce the limit, it won't do anything to existing avatars that are larger than the limit.
