When you go to vboptions > human verification manager > image verification library, did you select the verification type with an image next to

No test image is displaying in the Image Settings for GD, so it is most likely not installed on your server. Contact your host.

GD does not work. You would have an image displayed there if it worked - like this:

No test image is displaying in the Image Settings for GD, so it is most likely not installed on your server. Contact your host.

The most likely cause for a blank image is that you're GD doesn't have freetype2 support and you were using the "simple fonts" option previously. Support for "simple fonts" was dropped from 3.7.0 as it is unreliable for stopping bots.
