I get people signing up with ads in there name like useviagra Uttessepitece I get some strange ones but I guess there is not much you can do.

Add the "bad" bits to the banned usernames list. However, you might like to try and stop them from registering in the first place. (Have a look for spam prevention modifications, like "Is Bot", etc.)
