A little more info would be nice. What image are you doing it to?

for example th image https://vborg.vbsupport.ru/external/2011/07/5.gif
will have no alt
it will appear on page source as <img src='https://vborg.vbsupport.ru/images/editor/insertimage.gif'/>
I want for example to be <img src='https://vborg.vbsupport.ru/images/editor/insertimage.gif' alt='Thread title in image alt attribute'/>
so that the thread title e.g: Thread title in image alt attribute
will show on the image alt attribute

There is no option you may just turn on to have that happen. You'll have to edit the templates or code wherever you want to change it to be that way.

You'll have to search for each template you want to change. Go to a page where you want it changed and do this to find the template - vboptions > General Settings > Add Template Name in HTML Comments > set to Yes . Then go back to your page and view the source code and you will see the name of the template called around your part of the code.
