Hi, it was very helpful of you. The look changed to what I wanted but I didn't see the extra fields such as Supported, Version etc. Where can I get those?

You need to add things like that yourself. You'll need to make a form for users to fill out to input that data (similar to what they do here). The actual mod used on this site is not available to the public, so you'll have to write the form yourself. There is a Form Hack that you can use.

There must be a very similar solution because i posted a site which used it and they didnt edit it.

They didn't edit what? Chief First Post does not spit out those fields. You can't spit out those fields unless you have the data entered first. So, you need a method to enter the data so it can be put into the post.

The modifications on this site are all very tightly wound together. It isn't a matter of just clicking a button and having a modification all neatly ready to be offered to the masses. The author(s) of some of the modifications on this site have decided to not try to figure out how to split all the mods apart and make them work independently of each other so that they can be offered for download. At least, that is what I understand about them. Some of them have been offered up for download - just check out Princeton's lists of modifications, but some of the more complicated mods are just too hard to separate out from all the others or do what is necessary to make them generic enough to work for others.
