Easiest way...... find this line in your navbar:
$template_hook[navbar_quick_links_menu_pos4]And right above it, add this:
<tr><td class="vbmenu_option"><a href="http://fs18.formsite.com/Laughingdoll/form803569333/index.html" rel="nofollow">TTL Staff Application</a></td></tr>
