I have removed the following BB Code from the BB Code manager. However, it still continues to show up in the post. How do I completely remove it?

Have you tried asking the mod's author?

Did you use any template edits to the editor template? Oh, and check and remove any plugins related to the mod from the Plugin Manager, since they might not have been properly uninstalled.

I honestly don't remember where I got it from. I did check the plugin manager and found nothing. I also checked my includes folder just to see and there was nothing there.

Which template would it be for the posting (As pictured above)? Do you know?
