Submitting more info on "Contact Us"

Does anyone know how to modify the Contact Us page to submit custom input fields and info about the visitor's browser, operating system and flash version? Is there a mod out there that does this already?

I have not seen anything like this for the Contact Us page. I have seen mods where they detect the browser and show it in the Who's Online list. I suppose you can look at how they did this.

If you have the proper scripts you can edit the contactus template of each style and add what you need.
