Change username addition to details

How do I setup where it says to "Edit Email, password" but also include Change Username..

one place I saw had a place for a new username and a username confirm below where you would change password.

Kinda like letting the users have a chance at changing it vs emailing or sending a message to a admin. I believe with this hack there would be a way to set it to a max limit on days you can change it like a week or two before you can change it again.

Anyone know how to set this up?

i think it would be better if letting users Request Username (https://vborg.vbsupport.ru/showthread.php?t=149377&highlight=username) instead of allowing them to change usernames everyday !!

Thanks..I did search for it but it seems I may have missed it somewhere.
