Seeing as you didnt post a link as requested, globally disable all your hacks and see if the error goes away.

Perhaps a test account name/password so we can actually see what is happening.

As you are still on 3.6.4 (i suggest to upgrade to latest stable), this is caused by a modification that runs a plugin at the 'newreply_form_reviewbit' hook location. Disable or update this plugin/product.
