Vbsurvey suggest hook location

I just installed Vbsurvey in to our new VB forum. I'm unclear as to how to set the code in our forum. I understand that I need to assign a hook location for the code, but, I'm not sure which area makes the best sense.

Does anyone have a recommended hook location for a general topic survey?

Second question is once I have the hook location set, how do I initiate the mod? When I go to the forum area that is supposed to have the code will there be a option to start a survey?

Sorry for the simple minded questions, I'm new to all this.

See the vbsurvey tab on the navbar? That's what I'm trying to do, can you tell me how to do that?
