Hello. How can I add fields to the Contact Us form? OK, so I know how to actually "add" the fields, but what do I need to edit in order for them to actually be sent in the email?
And, while we're at it, I would like to edit the format of the email that is sent. I would like the email to look something like this:
The following message was sent to you via the Website Contact Us Form.
Name: NAME
Email: EMAIL
Phone: PHONE
Subject: SUBJECT
Message:
--------------------------------
Message Goes Here.
--------------------------------
Referring Page: http://whatever.com
IP Address: N/A
User Name: USER NAME
User ID: ID
