Adding IMAGES by THREAD topics?. Possible Codeing v. Pics!

I need to add another section for a small 45x45 image of the product in the thread, how can I do this anyone know any codeing or something I can do to get it like that. Very much appricated, if anyone can get this mod/stlye hack.
