Issue with "Who's Online"

For quite some time now, we have a problem when viewing Who's Online.
The problem is that, almost all the locations dont work. For example we get a lot of"Unknown Location - /forum/clear.gif " instead of the actual location

Dunno if it was caused from a mod installed but can someone help us on this?

did you try the "deactivate products" thingy ?!

... you deactivate all your products, and check if it is ok... then reactivate product by product to find the wrong one.

I was going to say referral spam, but if its Registered Users and Guests, i'm stumped.
