Any idea's on how this is done?

I am in the process of changing server's and cleaning up and changing my forum. Does anyone know how this was done.

The one that has the drop down area has where a user can choose Daily, monthly, other.

If anyone has any idea I would be so greatful to you. Thanks in advance for any help
