how do i make a custom page on 3.6

i want to basically have a php file that calls the complete page with no content in the middle. and i want to to be a file not a template.

example - www.mysite.com/test.php (http://www.mysite.com/test.php) where that file will do exactly what the pther pages do. and i can add what i want. this way of making a page doesnt allow me to call my js file. i tried giving it the direct url as well and nothing.

for example

no what you have is a template type page we need the hard code php
