Feedback Hack for 3.6

Hey guys, I was wondering if there is a Feedback Hack (like you know ebay feedback) Since were running a forum that is doing business etc. We want to be able to have user feedback but I can't seem to find anything that relates which is for vb 3.6. Can you guys help me out?

There is a "contact us" link at the bottom of the forum. The contact form allows you to select "site feedback", "registrations problem" or "other"

However this may not be what you are looking for...
