How Do I add another option?

Hello, I am trying to modify to sendmessage.php file. I am trying to add another field where the end user can type in their "Forum Link" kind of like it currently does for Email and Username. Can someone please tell me how to add that field?
I tried modifying the template and the file but when I do it doesnt get emailed to me.

I would also like to add a few extra fields. for Vendors, Advertisements and so om, each will have there own e-mail and or subject, so they can be sorted in my e-mail program. So not to get lost in all the SPAM.
