i did revert my header template but noting effect. now my header is not shown

it gives this error all forums, pages ect.

i had an error like that in different template (i don't remember which was that)

we add new plug-in, select
hook location : cache_templates
and fix that...

i think, add new plug-in again. but don't know what i write to "Plugin PHP Code"
