Duplicate post error... [Archive]

When I try to make a post, it tells me that it is a duplicate even if it isn't. The post still goes through, however. Help is needed asap.

My site is www.forumfatale.com and it happens on my site, not here. I don't have any duplicate post hack installed. Also, I disabled all hacks at one time and it still did it.

Have you made any file edits to newreply.php?

I was getting this on one of my servers, I upgraded php and rebuilt apache as well as upgrading APC to 3.0.10. Since then the issue has gone away.
