Help me to create a new database

I'd like to create new database but dont know what to put in the field box, please help

It appears you have a new database named "coolphim" with no tables in it.

If you are installing a script that needs an empty database to install, supply it with the db username and password as prescribed and it should fly.

Like noppid already replied, you already seem to have created a database, and are now trying to create a table in it.
