Adding Custom PHP Page Question

Hi all

I'm damn sure that this has been asked many times before, but I've not been able to find any references to it.

I have a custom PHP page which includes a form (Found here (http://ccgi.straightliners.force9.co.uk/community/weather_centre.php) if anyones interested). How can I include this into vBulletin 3.5.4?

I've tried using the GTCustom hack, but that kills the form. I don't want to do anything wizzy with it, just have it appear as if it was another vBulletin page.

It explains everything about creating custom vBulletin pages and it's very easy to understand.
