Anyone have idea on whats causing this

The cause will be a hack/plugin you have recently installed.

Do you happen to have the plugin for one sig per thread page installed? If you do then the odds are that is the problem especially if you are moderately to heavily modified as it appears to clash with some other mods. Basically I had the same problem for a client and found after much searching that that was the problem. Remove the plugin and your site will work correctly again.

Hah yep i was about to post and say that ive found what was causing it. and it was indeed the one sig per thread :)

If I'd only got here 10-15 mins earlier I'd have save you hassle of finding it yourself.
