construct_edit_toolbar: bbimg and title tags

I've using this code:

to construct an input box. It works great except for two items.

I can't figure out how to get the image link button to show
I can't get the pop-up title to work when I hover over an item on the toolbar.

Okay, I figured out that the popup title tags weren't working because I didn't cache the 'posting' phrasegroup.

But I still can't figure out why the button for the [IMG] tag won't show.
